NACDD Launches Communities of Practice Groups
Community of Practice Groups are new, cross-cutting, member driven groups that focus on single topics that apply across all Councils or a focus on a common topic of interest. Practice Groups are intended to last for 9-12 months and the Cardiovascular Health Council has recently launched three such groups. One of them is exploring
how to address hypertension by focusing on health information technology, meaningful use of electronic health records, and patient-centered medical homes. The group created a summary of their discussions, Improving the Quality of Care through Electronic Health Records, which includes a set of recommendations from a guest expert, many resources and tools, and information on states' activities. The CVH Council hopes to form new cross-Council communities of practice in Autumn 2011, to more fully explore these issues and the role of the chronic disease programs.
